Check. Change. Control – Partnerships that work!
Recently, the Live Healthy Vineland (LHV) coalition formed a new partnership with the American Heart Association (AHA). Through this partnership, LHV has been able to hold classes and provide “Heart Bucks” through the Corner Store Initiative (another area of focus for LHV). In November, LHV and the AHA began to implement “Check. Change. Control.” at our worksites. This program, through the American Heart Association, is an evidence-based hypertension management program that utilizes blood pressure self-monitoring to empower participants to take ownership of their cardiovascular health. The program incorporates the concepts of remote monitoring, online tracking and recruiting local volunteer health mentors to encourage participants.
We are excited to begin this program within our worksites as high blood pressure is an area of focus in the Worksite Wellness toolkit. This program will enable worksite leaders to encourage their employees to participate, with minimal follow-up and resources that need to be utilized. “Check. Change. Control.” has been implemented at three worksites so far, with the goal to incorporate into all of the worksites by end of year.
